This place is awful! My father went here after we were promised by the discharging hospital that is was a rehab facility and he would be on a rehab floor, all lies. He was put in with nursing home patients, the room was filthy with stale air and water stains on the ceiling. The "beds" are no more than 1" thick, the reclining chairs they give some residents are ancient and not even the aides knew how to work them.
On multiple occasions medications were 2+ hours late being given and one time my fathers medication was dispensed to him by a NURSE'S CHILD that he brought in for the day...a CHILD was handing out medication, yes the father/nurse gave it to them but that is ILLEGAL, my father's roommate got left on the bed pot after defecating for over 45 minutes and it took multiple times of us asking for them to clean him up so we could go back in the room and visit with my father. Anytime the call "bell"(NOT a bell, simply a light that lights up outside the room, it doesn't notify the nursing station) was rung it wasn't ever responded to in less than 20 minutes ever. The aides were rude, insensitive to the patients and the family and on many occasions I had to hunt them down for them to assist my father with anything after asking multiple times, they were always by the elevators in a large group chatting about whatever or outside smoking. The head nurses are just as bad, they sit at the desk and don't do ANYTHING except dispense the medication 2+ hours late. I asked a nurse 2 times to assist me with repositioning my father and she said she'd be right there, the 3rd time I asked she said she had told an aide, meanwhile she is playing with her cell phone at the desk and could have helped but apparently Candy Crush Saga is more important than helping patients.
All in all this place was a hell hole on earth and had we known how bad it was we would have looked elsewhere or brought my father home. I do not even want to give this place 1 star, it deserves SO many negatives. If I wrote all the examples down of how mistreated and ignored patients and their families are here I'd be here all day.

Bridgewater has a bad reputation due to events that have happened in the past. The facility is under new ownership and has undergone extensive renovations. Those who do take a tour are pleasantly surprised by what they see. The facility has a yearly review by New York State like all other facilities in the area and does very well (available for review). It is unfortunate that when a business changes itself for the better, the community does not come forward and support it. please take time to come tour and see Bridgewater Center for Rehabilitation and Nursing for yourself.

i have two family members at this facility currently and have had others there in the past. i agree with the Judith S. The place it disgusting, it smells bad all the time, it's really filthy and the residents do not get adaquate care. my aunt was put there for rehab purposes. she has only had 1 or 2 physical therapy sessions in the last year. when we ask why we get the usual understaffed explanation. now as a result of not getting the therapy, she can no longer walk and is loosing the ability to use her arms. my other relative is there and also has a bad situation. one example, she was made to wait over an hour to get someone to help her get to the bathroom. her stomach hurt so bad that she wasn't able to eat her lunch. someone should look into this place

I can state without a doubt that Bridgewater Center is NOT a caring rehab center. If you love your relatives than don't send them here. I know this because I lived here with my wife for eighteen months. During that time she got sick several times and was sent to the hospital with infections. The staff was so uncaring that I had to call the ambulance myself. The nurses refused to send her even when she had a fever.

As the administrator of Bridgewater Center for Rehabilitation and Nursing, I am stunned that Judith S. is allowed to print her libelous comments. There is no truth to them. In the very month that Judith S. recorded her comments, the NY State Department of Health inspected all aspects of this nursing home and found things in such a good state of compliance that there was no need for a return visit for a year. Bridgewater meets all fire and safety codes. Judith S. states there have been several owners - not true. RiverMede was owned by its orignial owner until August 1st, 2006, and the new Bridgewater Center owner and I, as well as the entire administrative team, are committed to providing the highest quality of life and care for elders and the disabled. Residents have always been cared for by Licensed Nurses and Certified Nurses Aides, and no one is left "to fend for themselves." No one is "hired off the street" to provide care or administer medications. All new employees undergo background checks by the NY State Police and previous employment histories are scrutinized. Narcotics are stored under double lock and are administered only by RNs or LPNs according to doctors' orders and state and federal guidelines. Food is fresh at every meal. Kitchenettes are stocked daily with fresh items in adequate quanity. Social Security, Disability, and other funds are handled according to NY State law in compliance with all government requirements. I say thumbs up for Bridgewater Center and a very big thumbs down for Judith S's unfounded comments.

Since August, 2006, Rivermede (Bridgewater) is under the care of a new highly skilled, experienced and caring administrator who is making sweeping changes in policy, attention to resident care including openness to family concerns, condition of the facility, training of staff, and overall operations. I know. She's my sister, and I couldn't be prouder of her lengthy record of facilities improvement, establishment of staff training and motivation programs, and extensive work in the field of nursing home reform. She is highly respected in her field and well sought after. Rivermede is fortunate to have her. In addition, having lost our own mother this year at the age of 92, the experience has added even more to her understanding of the needs and concerns of elderly residents and their families. Expect great things.

This Center has changed hands several times but has not changed in their care practices thoughout the years. Senior Citizens and disabled adults are housed here and are left to fend for themselves without supervision. Living in this unstable and dangerous place is not good and as an example I know first hand that medications including narcotics are distributed to residents by employees who are not licensed nurses but plain old people they hired right off the street to work at the facility.
The rooms are filthy and the dining area is also a roach trap. More than day old food is served to residents only the kitchen help are told to scrape the mold off some of the foods before they are reheated to reserve.
Snacks are supposed to be provided to all residents in the evening along with coffee, juices and sodas in the kitchenettes on each floor but lack in quantity and residents have to share one small creamer for three cups of coffee at times. Residents are made to sign their checks right over to the office when the Social Security and Disability come in. The residents may get 25.00 dollars out of thousand dollar rental checks each month for their personal use.
This is one of the biggest scams of a nursing home and patients are overcharged, go to bed hungry and have to worry about their safety because the place is such a firetrap.
