I first became a patient of Dr. Funches, in January 2008. She was a most wonderful doctor as were her staff. A few months later Dr. Funches retired and Dr. Arevalos took over her office. Dr. Arevalos is not the most personable doctor but I couldn't complain too much about him. His staff, and particularly his Nurse, are the rudest and most unprofessional people I've ever dealt with. I suffered an injury to my shoulder on a Sunday in May 2010 which caused me great pain in my shoulder and in my arm down to my hand. I went to an urgent care facility but there was nothing they could do. They referred me to the emergency room at MRHC. I didn't go to the emergency room that Sunday because my insurance won't cover the cost if life, limb, or eyesight are not in peril. Monday morning I called Dr. Arevalos' office to see how soon I could get in. I was routed to a voice mail box where I left a message about my situation. I got no response until late the next day, Thursday, when the nurse called and told me she was scheduling me for an MRI. I was pretty upset that his office had not contacted me on Monday and I told the nurse that in no uncertain terms. She told me their office policy was that they did not have to respond to my call for 48 hours. I told her I didn't want an MRI but I wanted to see the doctor and that was the original purpose of my call. She got indignant with me and I returned it right back to her. Her last statement to me was I should find another doctor! Fast forward to June 3, 2010 - I get a certified letter from Dr. Arevalos' office, dated May 12, 2010, terminating care from him and all other doctors at Heritage Park Medical Center for "non-compliance of medical advice" which I never received. I never had a chance to talk to the doctor. The letter also stated Dr. Arevalos would provide emergency care for 30 days after the date on the letter. By the time I got the letter I had 9 days remaining for this "emergency" care. I have peripheral neuropathy from a botched spinal surgery 8 years ago and I'm on non-narcotic maintenance medication ever since to relieve the pain in my hands. My prescription runs out 17 days after the letter I received and try as I might I've not been able to find a doctor I can see before that prescription runs out. Without that medication I cannot function at work because my hands hurt too much. I have not been able to discuss anything with Dr. Arevalos about any of this. He knows nothing of what my injury was, takes only the word of his nurse, and terminates my care. And then also writes I can't receive care from anyone else at Heritage Park Medical Center. I would not recommend anyone choose this doctor, his office, or Heritage Park Medical Center for their care.

Why would you depend on your child to give this PRESCRIPTION to the school? What is wrong with you? You know what your child is and is not capable of and yet you're blaming everyone, the doctor, your child, and the school but you never say "I should have....." Don't blame the doctor blame yourself

My 15 year old with an Autism Spectrum Disorder was given a doctors note for school excusing her from school. She carried the note in her backpack for 2 weeks and got it out in class and traced it with a dark green sharpie so she would not forget to turn it in. She forgot anyway. 2 weeks later at an IEP meeting with 7 other school personnel present, I retrieved the note from my daughter's backpack and carried it to the office. The secretary wanted to know why it was traced, I replied I do not know but my daughter has had it for 2 weeks and forgot to turn it in. 9 days later I receive a certified letter from Dr. DeLeon dismissing my daughter as a patient stating I (the parent) had altered a document. I was shocked at this behavior. He also states in his letter that I cannot see any of the doctors at this facility. This is unprofessional conduct in that there was no HIPAA consent signed allowing the school or doctor to communicate ANY information regarding the patient. Dr. should have called mom to ask if I had any knowledge about this. A 15 year old with Autism spectrum disorder did not realize what she was doing.
