Was seen for an annual physical by Padmini Gunadeva, MD on 8/15/2011.

1. The person who took my vitals was not a nurse. If so, he did not have on scrubs or a lab jacket. I did not see him wash his hands. He took my weight and blood pressure only.

2. Other patient??s urine was open in the bathroom were samples were left. (no lid).

3. The nurse who did the EKG had bracelets on up to her elbows and had long acrylic nails. I did not see her wash her hands before she put the adhesive tabs on me for the EKG. The exam table step had a visible stain and I did not want to put my bare feet on the floor because it was not clean either. As I looked around the exam room after sitting naked for over 30 minutes, there were stains on the walls that looked like blood.

4. The icing on the cake was Dr. Gunadeva. She clearly did not read the medical history questionnaire that I completed as she did not address any concerns that I listed ?? not one. She did not check my eyes, ears, look in my throat or take a pulse.

She performed a substandard breast exam. I am very fibrocystic; If she had look at my file, she could have seen the results from last??s year??s mammogram that was perform at their office.

The Pap smear was nothing less than torture. She did not have an assistant help her, nor was an assistant in the room. She scraped so hard and for so long that I thought the instrument was going to come through my stomach. She did not dispose of the instruments she used for the Pap. When I left, the instruments were left on the table uncovered.

5. Lastly, when she wrote my prescriptions, she was going so fast that she left 2 off. She gave me a 3-month supply of the meds which I did not notice until I left the office. I work a good distance from the office and asked her nurse to give me a least a 6-month supply. With mail order suppliers, it is just more simple and cost effective to have them written for more than one 90 day supply. I have been trying to reach her at her office for the last week to get an explanation for this and she refuses to come to the phone. Her nurse asked me to put it in writing.

I have been on the same meds for over 10 years with no problems. Why would I need to come into the office every 3 months just for her to give me the same thing? I think it is just a way to milk poor uninformed patients out of their hard earned money.

The care provided was substandard, Her ??drive-by? exam methods may lead to undesireable consequences.
